• Toe, Foot, or Leg Amputated Due to Illness or Disease
• Heart Attack or Stroke Within Last 2 Years
• Cancer Treatment (Other Than Non-Melanoma) Within the Last 2 Years
• Bedridden
• Currently Hospitalized
• Nursing Home Resident
• Required Oxygen
